Corporate Cup Challenge
ITEMS TO NOTE
- Teams will be comprised of no less than 6 and no more than 15 persons
- Team members must be employed by the sponsoring company(s)-- The cost per team member is $20 - Make check payable to Bismarck Mandan Chamber of Commerce Foundation
- All team members and participants MUST sign above as a release of liability before particpation
- No changes can be made to the Roster after April 13, 2012 without authorization from the Corporate Cup Committee
- 50 Bonus Points will be awarded to all teams with their Registration Form, Roster and Entry Fee submitted by April 1, 2012
CORPORATE CUP 2012 RELEASE OF LIABILITY STATEMENT/INFORMED CONSENT
I understand that in rostering myself for participation in the Bismarck/Mandan Corporate Club Challenge (hereafter the "Corporate Cup") I fully understand that specific activities of the Corporate Cup require athletic skill, and physical injuries, such as falls, sprains, strains, bruises, shortness of breath, etc., may result from participating in the Corporate Cup. I release and discharge all sponsoring persons or entities, their members, agents, employees, officers, and administrators from all liability for any damages I might sustain as a result of my participation in the Corporate Cup. I agree to seek compensation from my own health insurance carrier for any injury sustained as a result of my participation in the Corporate Cup. As a member of this roster, I have read and fully understand the above details and waiver and release all claims.
